It really depends on your bandwidth and ISP. Sometimes it can be garbled or sound like someone is underwater.

I have used Skype frequently, and it is always really clear – better than a regular phone usually. (Skype is free VOIP software for your computer).
I have experienced VOIP from Comcast…excellent sound and service. Now, Verizon FIOS. Also extremely good. Never had a problem.
